WHEREAS the Deceased died at PLACE on DATE at the age of AGE years, having made and executed his/her last Will dated WILL.  The Deceased appointed EXECUTOR as the sole executor/executrix in the Will.

 

NOW I, EXECUTOR, POSITION3, at present residing at ADDRESS3, do hereby nominate, constitute and appoint NAME, POSITION, of ADDRESS, to be my lawful attorney for the purpose of obtaining letters of administration (with the Will annexed) of the estate of the Deceased, to be granted to *him/her by the COURT of DOMICILE for my use and benefit, limited until further representation be granted, and I hereby promise to ratify and confirm whatever my attorney shall lawfully do or cause to be done in the premises.
