I, NAME, POSITION, of ADDRESS, do hereby make oath and say as follows:

1. I am one of the subscribing witnesses to the last Will of the Deceased, the Will bearing date WILL, being now produced to me and marked “A”.

 

2. The Deceased executed the Will on WILL, by signing his/her name at the foot or end thereof as the same now appears thereon, in the presence of me and of WITNESS, the other subscribed witness thereto, both of us being present at the same time and we thereupon, that is, after the Deceased had so signed, attested, and subscribed the Will in the presence of the Deceased.

And I make oath and say that the contents of this affidavit are true.
