I, NAME, POSITION, of ADDRESS, do hereby make oath and say as follows:

 

1. I knew and was well acquainted with the Deceased for many years during [his/her] lifetime.

 

2. During such a period I have frequently seen *him/her write and also subscribe *his/her name to documents, for which reason I have become well acquainted with *his/her handwriting.

 

3. Having carefully examined the paper writing now produced to me and marked “A” purporting to be the last Will of the Deceased, bearing date , I verily believe and confirm [the whole of the said Will, together with] the signature subscribed thereto to be of the true and proper [handwriting ] [and] [signature] of the Deceased.

 

4. I am now AGE years old.

And I make oath and say that the contents of this affidavit are true.
