1. I am the sole [executor/executrix] named in the last Will of the Deceased, the Will bearing date WILL, being now produced to me and marked “A”.

 

2. Having carefully examined the Will and particularly noticed (here recite the obliterations, interlineations, corrections, and alterations, if any, or describe the plight and condition of the Will, or any other matters requiring to be accounted for, and set forth the finding of the Will in its present condition, and, if possible, trace the Will from the custody of the Deceased in [his/her] lifetime to the time of making the affirmation/affidavit), I confirm that the Will is now in the same plight and condition as when it was found (or as the case may be) by me as aforesaid.
