I, NAME, POSITION, of ADDRESS, do hereby make oath and say as follows:

1. I am one of the subscribing witnesses to the last Will of the Deceased, the Will bearing date WILL, being now produced to me and marked “A”.

 

2. The Deceased executed the Will by signing *his/her name *[at the end thereof], as it now appears thereon, in the presence of me and of C.D., the other subscribed witness thereto, both of us being present at the same time, and we thereupon attested and subscribed the Will in the presence of the Deceased.

 

3.
