To the COURT of DOMICILE:

WHEREAS the Deceased died at PLACE on DATE, having made and executed his/her last Will dated WILL.  The Deceased appointed EXECUTOR as the sole executor/executrix and trustee and named RESIDUARY the residuary legatees and devisees therein. 

   By a renunciation dated ................. EXECUTOR, the sole [executor/executrix] named in the Will, renounced all [his/her right] and title to probate and execution of the Will.

AND WHEREAS by a letter of authorization dated AUTHORISATION, RESIDUARY2, one of the residuary legatees and devisees named in the Will of the Deceased duly authorized, requested and consented to NAME, of ADDRESS, to apply for letters of administration (with the Will annexed) to be granted to them by the COURT of DOMICILE in respect of the estate of the Deceased.

NOW I, RESIDUARY3,  POSITION3, of ADDRESS3, one of the residuary legatees and devisees named in the Will of the Deceased, hereby consent to letters of administration (with the Will annexed) being granted to (name of the trust corporation) in respect of the estate of the Deceased.
