Dear Sir / Madam,

Complaint of Harassment by the Occupant of ADDRESS

I ACCOUNT_FIRST_NAMEACCOUNT_LAST_NAME, am the Occupant of ACCOUNT_ADDRESS_SINGLE_LINE. I would like to lodge this formal complaint against NAME the Occupant of ADDRESS (the “Harasser”), who has been harassing our family for the past month.

[PLEASE STATE THE NUISANCE / HARASSMENT - In [Period], we received numerous complaints from the management office that we were too noisy, so we kept close attention to any noise generated from our unit. However, the complaints did not seem to go away and the Harasser would ask the management office to ring our bell up to 3-5 times a day telling us that we are too noisy, when we are only carrying out our daily routine activities. It was causing a lot of disruptions to our daily family lives. When we asked the management office to stop ringing our door bell and arrange to speak directly to the Harasser, the Harasser refused to meet with us.]

[PLEASE STATE WHAT PROMPTED THE LAST STRAW - On the night of [Date], two police officers knocked on our door without warning and told us that the Harasser has called the police department to complaint about our noise. We were at that time carrying out no more than our daily activities - having dinner and revising for tests. When we asked the police officers if they were hearing too much noise from our apartment, they confirmed that they have been listening outside for a long time and the noise level from our apartment is similar to other apartments in the building.]  

[PLEASE STATE DAMAGES CAUSED - Our two boys were terrified by the police visit and had nightmares last night. The incident has caused them a lot of emotional distress and we may need to take them to a psychologist for check-up. The incident has also caused stress and annoyance to our whole family.]
