Dear Sir / Madam,
Complaint of Discrimination by NAME
I would like to lodge this formal complaint against NAME, who is in the position of POSITION (the “Bully”) under the Company's [Human Resource] Policy. My complaint is that NAME has engaged in conduct on/between PERIOD in breach of the [Human Resource] Policy.
[DESCRIBE THE FACTS] Ever since I joined the company in [Month] NAME has treated me in a condescending manner.
[On [date] at about [time], I was at my desk working when NAME said [words to the effect of] “[STATEMENT]”.]
[On [date], NAME sent an email to me alleging that "[ALLEGATION]". A copy of this email is attached and marked A. I was very distressed by this email.]
[NAME frequently commented on my background in relation to my [race/gender/religion/socio-economic background/disability].]
[NAME has consistently behaved towards me in a manner which not only inconsistent with nor reflective of the value of the Company.]
[I have tried to speak with NAME privately to address the issue but [he / she] has been avoidant. On [date], we finally had a one-on-one meeting, [he / she] refuses to admit that [he / she] has conducted themselves inappropriately and bullied me. [He / She] claimed that I was the one reacting to criticisms too sensitively. The bullying behaviour have not stopped after the meeting. When I pointed them out, [he / she] said they did not have ill-intentions and refused to apologise even after I said their conduct was inappropriate.]
