Re: Request for Maternity Leave

I am writing to let you know that I have completed NUMBEROF months of pregnancy and hence applying for maternity leave effective from DATE. As per the doctor's advice,  my baby is expected on DUEDATE. In accordance with the company's policy, I wish to take PAIDWEEKS weeks of paid maternity leave. A letter from my doctor confirming the relevant details has been attached herewith.
