I am issuing you, NAME with a final cease and desist letter on the unwelcomed sexual advance and harassment actions towards or against me, including but not limited to the following:

☐  Inappropriate staring, leering or wolf whistling.

☐  Stalking or obscene exposure.

You must immediately cease the above unlawful actions of harassment against me which violates my right under the law. As a result of your actions, I have suffered the following damages: 

☐  Severe emotional and mental distress.

☐  Physical assault and harm to my body.

☐  Damages to my professional reputation.

☐  Damages to my employment as I was unable to focus on my work during working hours.

☐  Defamation of my character.

☐  Insomnia due to being frightened and intimidated.
