1. Purpose
ACCOUNT_JOB_COMPANY (the “Company”) the employees will need days off from work time to time to address their medical needs. The purpose of this policy is to outline the company’s criterion and procedure for the employees to avail paid sick leave (“Sick Leave”). It sets out the terms for eligibility criteria, how to apply, accrual of Sick leave etc.
This policy is applicable only for Sick Leave and does not include any other type of leave such as maternity leave.
2. Eligibility
(a) Employees are entitled to apply for Sick Leave after completing NUMBER months of continuous employment.
(b) Employees are entitled to full pay sick leave subject to a maximum of the lesser of 120 days or the number of sickness days that the employee has accumulated.
(c) If the employee has exhausted all the accumulated Sick Leaves and requires additional leaves to attend to his health, then the employee can request for leave without pay.
3. When to Apply?
An employee can apply for sick leave when they are unable to perform their regular duties due to illness or injury/accident or need to obtain professional services of health care practitioners including dentist/physiological care. The employee may apply for half-day or full-day of Sick Leave.

The employee must notify their immediate supervisor when they want to use their sick leave with the expected date of return to work. The employee understands and acknowledges that the Company, at its own discretion, may require a medical certificate for leave to grant any Sick Leave request.
