The appraiser should put his numerical rating of the skill in the 'appraiser rating' column. The appraiser should offer an explanation for giving this rating in the 'notes' column. The employee and appraiser can comment make further comments on this rating in the 'employee comments' and 'appraiser comments' rows respectively.
