Re: Request for Adoption Leave

I am writing to let you know that my spouse and I are in the final stages of adopting a child. We are expecting that the child will be placed in our household by DATE. Please find enclosed the supporting adoption documents for your perusal.

We have decided that I will be the main adoptive parent. Thus, I am formally applying for adoption leave effective from the date of placement of the child in our household. As per the company's policy, I wish to take PAIDWEEKS weeks of paid adoption leave.
