This Child Custody Agreement ("Agreement")  is made on 29 April 2024

 

BETWEEN:

(1)    PARTY_1_NAME (“Father”) of PARTY_1_ADDRESS_SINGLE_LINE; and

 

(2)    PARTY_2_NAME (“Mother”) of PARTY_2_ADDRESS_SINGLE_LINE

 WHEREAS:

(A) The Parties are the parents or legal guardians of the following minor child (ren) (the "Child"):

Name: CHILD_NAME, Date of Birth: CHILD_BIRTHDAY

(B) The Parties hereby confirm that no other minor Child now living has been born during the marriage. 

(C) The Parties have agreed to enter into this Agreement to settle the terms and conditions relating to the care and custody of the Child. 

NOW THEREFORE, the Parties freely and fully accept the provisions, terms and conditions hereof and in consideration of the promises and mutual covenants herein contained as well as for other good and valuable considerations not herein specifically set forth, the Parties do hereby agree to the following:

1. Legal Custody 

The Father shall have the sole and exclusive legal custody of the Child and shall be free to make all decisions in all matters pertaining to the upbringing of the Child, including but not limited to, matters of health, education, medical and dental care and religion. The Parties agree that granting sole custody to the Father will not deprive the of access to and information regarding the Child.

2. Physical Custody 

2.1 The Parties agree that the Father shall have the primary physical custody of the Child with reasonable access to the Mother in accordance with the terms of this Agreement. The Child will reside with the Father at the residential address PARTY_1_ADDRESS_SINGLE_LINE and the Father shall be free to make all routine and day to day decisions regarding the Child.
