MISSING MILEAGE REQUEST

I attach my missing mileage request form for missing mileage from my SERVICE in MONTH this year.

I hope you can credit me with the missing miles. [This flight back to the UK was arranged very quickly because I received a phone call to say that my mother was ill and was being taken into hospital. I arranged the flight and flew home very quickly－I remember my travel agent telling me that if I paid a few hundred dollars extra, my flight would be eligible for air miles, so I chose to do this. However, when I got to the check-in desk at the airport, I was very upset and anxious, and I don't recall reminding the check-in clerk that I was a CLUB member. Perhaps this is why my account has not been credited with the extra miles?]

As you will see from my SERVICE receipt, I did mention that I was a CLUB member when I picked up my hire car at AIRPORT airport. However, again these miles have not been credited to me.

I hope you can look into this and that I will be credited with the air miles due from both my air travel and my hire car rental.
