Parent / Legal Guardian’s Information

 PARENT_INFORMATION

I, the undersigned, is the parent or legal guardian of the child named hereinabove (“Child”), hereby consent to the participation of my Child in all activities and/or events conducted by (“Activities”) and certify that my child is fully capable of participating in the Activities and that my Child is healthy and has no unknown/hidden physical or mental disabilities or infirmities that would restrict his/her full participation in such Activities.

Further, I hereby declare and agree:

1. Assumption of risk and Release — I expressly assume all risks associated with my Child‘s participation in the Activities including the risk of injury or harm to the Child and am willing to take the risk. I hereby release the FULL_NAME, their management, employees, volunteers, directors, officers, representatives, agents, organizers and successors, from and against any and all monetary claims, liabilities (including liability for any personal injury) demands, actions, causes of action(s) whatsoever, at law or in equity, arising out of my Child’s participation in the Activity as herein provided.

2. Covenant not to sue — I promise not to sue FULL_NAME should my Child suffer from any damage, loss, personal injury, or death as a result of my Child’sparticipation in the Activity.

3. Waiver of liability — I waive the right to hold the FULL_NAME liable for any liability, cost and/or damages that may arise from my Child’s participation in the Activity.
