3. Terms of Use

a. I understand that the equipment listed herein above (the “Equipment”) is the property of the ACCOUNT_JOB_COMPANY (the “Company”) and I’m authorized to use it for business purposes and in the course of the proper discharge of my duties as an employee of the Company.

b. I understand that I will report any damage, loss, or theft of the Equipment immediately to the DESIGNATION

c. I shall be responsible for the cost of replacing the Equipment in the event of any loss or damage to the Equipment from the time I take possession of the Equipment until it is returned to the Company

d. On or before the date of termination of my employment with the Company, I shall return all the Equipment to DESIGNATION. I shall be liable to reimburse the Company for any loss of or damage to such Equipment, except for fair wear and tear. The amount of such loss or damage shall be deducted from the final payment in compliance with the applicable laws

5. I shall comply with the Company’s equipment usage policy (if any) and understand that a violation of the terms and conditions set out therein will result in the restriction and/or termination of my use of the Equipment and may result in termination of employment and/or legal action.

By signing this Agreement, I acknowledge that I have received the Equipment in working order and I agree to the terms of usage of the said Equipment listed herein above
