Terms and Conditions

ACCOUNT_JOB_COMPANY (the "Company") is in the business of  providing BUSINESS_DESCRIPTION 

I, as the Client of the Company, have enrolled myself and my dog in dog training classes, workshops or other related activities offered by the Company

Participation - I hereby consent to the participation of myself and my Dog in the activities and/or events conducted by the Company that will involve training classes, workshops and related services (“Activities”). The risk, hazards and dangers associated with these Activities are, including but are not limited to RISK

Facility and Equipment Inspection - I agree before I participate in any Activity, I will inspect the related facilities and equipment, I will immediately advise the coordinator of the Activity of any unsafe conditions that I observe. I will refuse to participate in the Activity until all unsafe conditions observed by me have been remedied. 

Medical Fitness - I hereby certify that:

(a) I am physically fit to participate in the Activities in which I have chosen to participate and have not been advised otherwise by a medical practitioner. 

(b) My Dog is healthy and fully capable of participating in the Activities and has no unknown/hidden physical or mental disabilities or infirmities that would restrict my Dog’s full participation in such Activities.
