I hereby grant and authorize FULL_NAME to use my photograph, video footage, image, likeness, and voice (collectively, the "Media") in any and all media, including but not limited to print, digital, and online platforms.
I understand and agree to the following terms:
(a) I grant the right to edit, enhance, crop, alter, modify, amend, copy, or otherwise make use of the Media for any for use in any manner as desired by FULL_NAME, without any consideration or other payment.
(b) I waive any rights to inspect or approve the finished product before distribution.
(c) I waive the right to any compensation, including but not limited to any royalties, arising from the use of any or all of the Media.
(d) I understand and agree that all Media is the property of FULL_NAME and will not be returned to me.
(e) I hereby agree to hold harmless, release, and discharge FULL_NAME, its heirs, representatives, executors, administrators, or any other persons acting on behalf of FULL_NAME from all claims, demands, and causes of action which I, my heirs, representatives, or any other persons acting on my behalf, may have by reason of this authorization.
(f) I confirm that this Release shall continue indefinitely unless I withdraw the consent provided herein. I can withdraw my consent at any time in writing by email to EMAIL at
