We would like to draw your attention to the behaviour of your staff member STAFF.
We are unhappy in the manner in which STAFF handled our order. [DESCRIBE THE BAD SERVICE: e.g. we were promised A but B was provided instead; the staff member was extremely rude when handling our order; the staff member did not complete the task as promised; the staff member was late in providing the deliverables, the representations made by your staff member were false and misleading etc.]
We hope that you will understand our position and pass the word to [him / her] on the issue.
