Re: Termination Notice

Dear FIRST,

This is to notify you that your employment with the ACCOUNT_JOB_COMPANY is terminated effective immediately. You will be provided with the standard severance pay for the notice period. Human Resources will contact you on your severance package after submission of the required paperwork. 

As we have discussed on various occasions, you have not been able to meet the basic requirements of your job and your performance has been below the company’s expectation. You POORLY

You gave us assurances each time that you would shape up and perform the duty you are hired to do but you have failed to deliver on your promise. I am sure you will agree that I have no choice under the circumstances but to terminate you.
