At the request of the Club, the club has to provide an enrolment form to record participation for club representatives in sporting/recreational/education events organised by the event organiser. The enrolment form is to contain details in (a) particulars of the respective activities including activity No., activity name and activity date; (b) particulars of the respective club organising the respective activities, including its name, correspondent address and contact methods; (c) particulars of the club representatives including their names, correspondent address and contact methods; (d) the rules and regulations that should not be violated during the activities; (e) the declaration of trust and confidence; and (f) condition of use and data collection protection clauses.
If the respective club representative agree all of the details and declaration, the participants must sign the document. The invitation is to invite proposed participants to the respective activities.
This document can be used as a template as a formal invitation to their club activities.
Details of the club activities should be included in the invitation. The invitation may also be customised by including the distinctive features of them and/or the club activities in the invitation.
All participants must not violate the rules and regulations, which are 1. obeying the coach and respecting others, 2. agreeing not to talk and shout during training, 3. not wearing any necklace, bracelet, earring, rings and ornaments during training, 4. not engaging in destructive behaviour and keeping the place clean, 5. not engaging in hooliganism in the name of the organisation, 6. not engaging in bullying, humiliating and persecuting, 7. not engaging in slanderous and insulting behaviour, and 8. not engaging in sparring without permission and fighting.
