This is a Deed Poll on Change of Name for an Individual. This Deed Poll needs to be signed in the presence of a solicitor or a notary public.

A deed poll is required in order to acquire a name change. Whereby the undersigned hereby absolutely renounces and abandons the former name, and in lieu thereof do assume from the date hereof the name set out ('Newly Assumed Name').

IN pursuance of such change of name as aforesaid, the undersigned hereby declares that they shall at all times hereafter in all records, deeds and instruments in writing and in all proceedings etc use and sign the Newly Assumed Name. They hereby authorise and request all persons to designate and address them by their Newly Assumed Name only.
