This is a general form of a Statutory Declaration to affirm the contents to be true and correct before a qualified witness. It is used when there are no court proceedings.
The individual solemnly and sincerely declares a statement. Any person who knowingly and willfully makes a statement false in a material particular, such statement being made in a statutory declaration, shall be guilty of an offence and shall be liable on conviction upon indictment to imprisonment and to a fine.
This Statutory Declaration should be signed by the individual who makes the statement in front of solicitors or a notary public.
