An Image Release Form, also known as a Photo Release Form, or Model Release Form, is a document used by individuals or photographers (known as a "Releasee") to obtain the consent of another person (known as a "Releasor") to use any image for personal or commercial purposes.
An image release form is used when photos or images of people are taken, and the photographer wants to publish the images. Consent, through an image release form, must be obtained before publishing such images. Such consent is necessary for the individual or photographer to avoid copyright infringement.
Under this specific template, the rights to these images are given free of charge - without any consideration or payment.
Furthermore, the Releasor, in this form, waives the right to any compensation, including but not limited to any royalties, arising from the use of the photos.
The Releasor understands and agrees that all photos are the property of the Releasee and will not be returned to the Releasor.
This grant of authorisation includes publications by the Releasee’s on mediums including, but not limited to, the Internet, e-mail, magazines, flyers, advertisements.
This document should be carefully read by the Releasor and Releasee. Both parties should sign and return a copy of this form. Once signed, both parties should get a copy of the signed form.
This image release form is suitable for adult signatories. This image release form is not suitable for children. In other words, if the consent of a child is required, this form is not suitable.
The Releasee should ensure that this document is signed by the Releasor before they publish the image.
To avoid any future disputes, both parties may wish to have their signatures witnessed.
