A Video Release Form, or Model Release Form, is a document used by individuals or videographers (known as a "Releasee") to obtain the consent of another person (known as a "Releasor") to use a video for personal or commercial purposes.
A video release form is used when videos of people are taken, and the videographer wants to publish the videos. Consent, through a video release form, must be obtained before publishing such videos. Such consent is necessary for the individual or videographer to avoid copyright infringement issues.
Under this specific template, the rights to these videos are given free of charge - without any consideration or payment.
Furthermore, the Releasor, in this form, waives the right to any compensation, including but not limited to any royalties, arising from the use of the videos.
The Releasor understands and agrees that all videos are the property of the Releasee and will not be returned to the Releasor.
This grant of authorisation includes publications by the Releasee’s on mediums including, but not limited to, the Internet, e-mail, magazines, flyers, advertisements.
This document should be carefully read by the Releasor and Releasee. Both parties should sign and return a copy of this form. Once signed, both parties should get a copy of the signed form.
This video release form is suitable for adult signatories. This video release form is not suitable for children. In other words, if the consent of a child is required, this form is not suitable.
The Releasee should ensure that this document is signed by the Releasor before they publish the video.
To avoid any future disputes, both parties may wish to have their signatures witnessed.
