Document Description

The Appraisal Form (employee evaluation form) is a document used for assessing an employee's performance and potential for promotion. It is an essential tool for evaluating employees and providing feedback on their strengths and areas for improvement. The form consists of several sections that cover different aspects of the employee's performance.

Section A focuses on assessing the employee's potential for promotion. It provides four options for evaluating the employee's current fit for promotion, ranging from 'not fitted at present' to 'fitted for promotion'. This section helps identify employees who have the potential to grow within the organization.

Section B allows the employee to express their career aspirations. It provides space for the employee to describe their interests, mobility, previous experience, and other relevant details. This information helps in aligning the employee's goals with the organization's objectives.

Section C evaluates whether the employee has the abilities to perform well in a job at the same grade but in a different discipline. It asks whether the employee displays the necessary skills and competencies for a different role. If the answer is 'yes', the form provides space to list the identified job(s).

Section D focuses on training and development. It asks whether further training or other development actions would be appropriate for the employee. If the answer is 'yes', the form provides space to list the identified training or development opportunities.

Section E is for the countersigning manager's comments. The countersigning manager confirms whether the employee has demonstrated suitability for promotion and explains any areas of disagreement with the reporting manager's assessment.

Section F allows the employee to provide their own comments on the assessment of their potential. It gives the employee an opportunity to share their perspective and provide additional insights.

Overall, the Appraisal Form is a comprehensive tool for evaluating an employee's performance, potential, and aspirations. It helps in identifying employees who are ready for promotion, determining their training and development needs, and fostering open communication between the employee and their managers.
