Misconduct Warning

This is a final written warning issued by an employer to an employee for misconduct. 

This letter begins by informing the employee of the rule that has been broken by them. It states the specific conduct that constituted the breach of the rule by the employee.

It also states that should the misconduct recur, the employee's employment with the company may be terminated.
