This document can be used as an appraisal scheme template. It sets out all the relevant factors to be taken into account in assessing/evaluating/judging the employee's potential/competency/strength/weakness.
An appraisal/employee evaluation is usually an annual process, whereby an employer assesses the performance of an employee. It is a key process for every business whereby an employer can highlight strengths but also areas of improvement for employees.
Appraisals are often used by employers to benchmark and compare the performances of different employees in a particular department. They are often also used by employers to decide whether to offer promotions, bonuses or other job-related perks to employees.
The appraisal scheme template provides allows the employer to score an employee's performance in eight key areas - the volume of work, job knowledge, safety awareness, dependability, teamwork, attendance and punctuality, work planning, and communication - and provide an overall marking.
Descriptors for each criterion guide employers in giving employees a performance rating. The template also features space for employers to provide detailed comments for each criterion. It also features sections for comments by the supervisor on that specific assessment, by the supervisor manager, and by the employee.
Lastly, there contains space to detail the agreed course of action for the employee in target areas of training or counselling requirements, career development and job and development objectives.
